In order for each child to play in the WYSL, certain items of documentation are required. The first time a player plays in the WYSL, a birth certificate is needed to verify the age of the child.
Each year, or when children change teams, a picture of the child is needed for a new player pass. As well, a medical release form must be given to the child's new coach. Medical release forms are also held by the training staff and the director of training in case a child is injured at practice.
Here are the specific instructions for documentation PLEASE READ THEM.
- MEDICAL RELEASE: Print and fill out the medical release form : Do NOT notarize the form. Do NOT use a different form. This form does NOT require a doctor's signature. DO NOT USE STAPLES.
If you attach copies of your health insurance card, please leave the copy on 8x11 paper. Do NOT cut out copies of the card. If possible, please copy both sides of the card on ONE SIDE of a 8 by 11 page (DO NOT copy on both sides of the page!!!). - ONE (headshot) picture of the player. The child's head including hair MUST BE NO BIGGER than the size of a quarter. PLEASE DO NOT SEND A HUGE PICTURE, your coach will have to send it back to you and it may delay getting passes for your entire team.
- Write the name of the child and the child's age group (e.g., U09 for third grade) on the back of the photo.
- BIRTH CERTIFICATE OR PASSPORT COPY. If this is your child's first time playing for an SYSC travel team, we need a copy of the child's birth certificate or passport. (all children registering for third grade teams MUST provide a birth certificate or passport). DO NOT MAKE A COLOR COPY OF THE PASSPORT or birth certificate (just do black and white), the copy is used for 1 minute to validate the birth date and then it is shredded.
ALL DOCUMENTATION MUST BE READY TO SUBMIT TO YOUR COACH AS SOON AS YOUR CHILD IS ASSIGNED TO A TEAM.
