Player Documentation Requirements

In order for each child to play in the WYSL, certain items of documentation are required. The first time a player plays in the WYSL, a birth certificate is needed to verify the age of the child.

Each year, or when children change teams, a picture of the child is needed for a new player pass. As well, a medical release form must be given to the child's new coach.  Medical release forms are also held by the training staff and the director of training in case a child is injured at practice.

Here are the specific instructions for documentation.

  • MEDICAL RELEASE: Print and fill out the medical release form (does NOT require any doctor signature): Do NOT notarize the form. Do NOT fill in your child's social security number. DO NOT USE STAPLES. 
  • If you attach copies of your health insurance card, please leave the copy on 8x11 paper. Do NOT cut out copies of the card.
  • ONE (headshot) picture of the player.  The picture should be no bigger than 2 in by 2 in, and the child's head should be about the size of a quarter - no bigger. PLEASE DO NOT SEND A HUGE PICTURE, we just have to reduce it, which takes time.
    Write the name of the child and the child's age group (e.g., U09 for third grade) on the back of the photo.

  • BIRTH CERTIFICATE OR PASSPORT COPY. If this is your child's first time playing for an SYSC travel team, we need a copy of the child's birth certificate or passport. (all children registering for third grade teams MUST provide a birth certificate or passport). DO NOT MAKE A COLOR COPY OF THE PASSPORT or birth certificate, it is used for 2 minutes to validate the birth date and then it is shredded.
